PRE-ADMISSION REQUIREMENTS
Submit the following documents for the Entrance Exam application:
1. Certified True Copy of Report Card (Form 138)
Transcript of Records (for Second Coursers and Transferees)
NOTE: The general weighted average is a very important factor in the admission procedure. The original Report Card/ Form 138 and/or Transcript of Records should be submitted as soon as possible to qualify the applicant for ranking especially in our quota courses.
2. 2 pcs. 2 x 2 Colored Pictures
3. Application and Testing Fee (P200.00)
(The Health Care Services Course Applicants should pay only P100.00 for Application Fee. They are not required to take the Entrance Exam and pay the Testing Fee.)
