In compliance with the Data Privacy Act of 2012 and its Implementing Rules and Regulations, I hereby allow Iloilo Doctors' College to collect, process and use all my types of personal information including those gathered through the Electronic Attendance Monitoring System and similar systems. The processing of personal information shall be done, subject to compliance with the requirements of Data Privacy Act, and other laws allowing the disclosure of information to the public or third persons entities with adherence to the principles of transparency legitimate purpose and proportionality. Processing of personal information is any operation performed upon data, including but not limited to the collection, recording, organization, storage, updating or modification, retrieval, consultation, use, consolidation, blocking, erasure or destruction of data. The College respects the right of its students and employees to privacy and aims to comply with the requirements of all relevant privacy and data protection laws, particularly the Data Privacy Act. It seeks to strike a balance between the personal privacy of its students and the free flow of information, especially when pursuing its legitimate interests and when necessary to carry out its responsibilities as an educational institution, employer, and/or as a contracting party. I agree and authorize IDC to continue to use my personal data where a legitimate education or institutional or institutional interest exists in its determination. To the extent permitted by law, mu information may be used by the University as an educational institution, contracting party, including a variety of administrative, research, historical and statistical purposes; admission, marketing, guidance services; compiling statistics and surveys and research for internal, statutory and reportorial requirements; submission of information to government agencies and other entities like accrediting, academic and business institutions; and for other purposes, when necessary and under circumstances permitted or required by law or regulation; and in any case, to share my information to third parties for any legitimate purpose. In the case of my application form, I agree that my responses and other data may be stored electronically in a local or cloud based storage of the College for a period of one (1) year and may be deleted should I decide not to pursue my education in the College unless said data is needed for other educational, research, statistical and historical use or purpose. This consent is valid until my graduation, and thereafter legitimate purposes like alumni concerns. By entering into this Agreement, I am not relying on any oral or written representations or statements other than what is set forth in this Agreement.
